GRANT APPLICATION PROCESS FOR SOUTH POINTE HOSPITAL (SPH), OTHER HOSPITAL OR HEALTH CARE FACILITIES WITHIN THE CLEVELAND CLINIC HEALTH SYSTEM
All grant requests submitted by or on behalf of South Pointe Hospital (SPH), the Cleveland Clinic, or any hospitals or health facilities within the Cleveland Clinic Health System must follow South Pointe Hospital’s internal grant application process and procedures and cannot be submitted directly to the Brentwood Foundation. To obtain information about South Pointe Hospitals grant application process and procedures, contact the Development Director at South Pointe Hospital at (216) 491-7234.

Grant requests from South Pointe Hospital or the Cleveland Clinic Health System that do not follow South Pointe’s internal grant application process will not be reviewed by the Foundation for funding consideration.

Organization Description
Briefly describe your organization, the nature of its work, and achievements as they relate to the needs you have identified.

Statement of the Problem
State the problem you plan to address and how it will assist the Brentwood Foundation in carrying out its mission.

Program/Project Description
Describe the program/project, including the activities and a timetable for the proposed activities, equipment or materials needed to address the stated problem and intended outcomes.

Funding Support
Discuss how funds will be used, organizational contribution both financial and in-kind and other committed or pending funding support for the project.
